In Order to Place Students Into Homerooms Each Year, Campbell Ridge Is Guided by the Following

<p> Grant’s Lick Elementary Classroom Placement Parent/Guardian Input Form</p><p>This form must be turned in to the office by April 30th, 2014.</p><p>In order to place students into homerooms each year, Grant’s Lick Elementary is guided by the following considerations:  Traditional grade level groupings for homeroom assignments  Equally mixed abilities and genders among homerooms as much as possible  Flexible groupings of students for instruction based on student needs</p><p>The Grant’s Lick Elementary Student Assignment Policy, as written by the Site-Based Decision Making Council, prohibits the Principal from considering a parent request for a specific teacher. However, the Principal may consider special factors when placing students into homerooms. This form allows parents/guardians to alert the Principal to special factors concerning their child. Please do not request a specific teacher on this form. If a specific teacher is listed, the form will be returned to the parent/guardian, and a new one will need to completed. </p><p>Examples of special factors include:  My child has an easy-going, “free spirit” type personality. If s/he is placed with a very strict teacher, that may not work well.  My child would benefit from a more structured classroom with a “stricter” teacher.  My child has struggled with math every year and might benefit from a teacher that has a track record of having special strengths in teaching math.  A teacher at this grade level is my sister-in-law, and I would prefer that my child not be in that homeroom.</p><p>Examples of things that are NOT special factors:  My child’s older brother had Mrs. Jones, and we would like her again.  My child asked me to tell you that s/he wants to be in Mrs. Jones’ class.  My child hates homework, and the rumor is that Mrs. Jones doesn’t give homework.  My child wants Mrs. Jones because she is the fun teacher.</p><p>My child’s special factors for consideration are:</p><p>______</p><p>______</p><p>______</p><p>______</p><p>______</p><p>______</p><p>______</p><p>______(Parent signature) (Student name) (14-15 grade level)</p><p> I have a unique situation that requires me to ask that my child NOT have a specific teacher, please call me at ______.</p>
