<p> HANDOUT 7-1 INITIAL SITE INVESTIGATION CHECKLIST PRIOR TO LEAVING Yes No 1. Ensure that the accident site is secured to prevent unauthorized people from entering and contaminating the scene. 2. Review Investigation Risk Assessment’s/JHA’s.</p><p>Team members who may need medication, i.e. bee sting kits, 3. have the medication readily available and other members have been briefed. 4. Team has the required PPE, transportation, communications, and water for each member. 5. Known or anticipated safety hazards at the site have been abated or otherwise mitigated. 6. The team has all required equipment, including the Accident Investigation Kit, and people before moving to the site. Adequate film/memory cards for all cameras are available, 7. adequate blank cassette tape for the tape recorders are on hand, and extra batteries for each are available. Contact the local agency or local law enforcement to ensure that 8. any available preliminary investigation information and/or special interest in the incident are known. The team is aware of the site security procedures that have been 9. established and will comply with them. Note: These may be changed by the team. INITIAL ACTIONS AT THE ACCIDENT SITE Yes No 1 Prohibit people not on the SAIT or invited into the site for 0. assistance from entering the site. </p><p>1 If there is evidence that may be easily disturbed, only the chief 1. investigator and only those he/she assigns should enter the area.</p><p>1 Approach the accident site from the same direction as it was 2. approached by those involved in the accident.</p><p>1 Establish the main task and flow of work. 3.</p><p>1 Get an overall view of the accident site before any evidence is 4. disturbed.</p><p>1 Determine the extent of the accident site; establish the physical 5. dimensions and boundaries. Mark them if necessary. </p><p>1 Control the entire site and protect evidence until it is released 6. back to the unit by the Chief Investigator. </p><p>DOCUMENTATION Yes No HANDOUT 7-1</p><p>1 All evidence collected is bagged and/or tagged and logged in the 7. evidence log; chain of custody is established and will be maintained. 1 All pictures taken are logged using the photographic evidence log 8. and identified on a sketch. </p><p>1 The accident site has been describe to ensure accuracy using 9. drawings, photographs, historical records, etc.</p>
