<p> Goshen Recreation Summer Day Camp Permission Slip to attend Field Trip to The Castle Fun Center in Chester, NY</p><p>I authorize my child (PLEASE PRINT)______to attend the field trip to The Castle Fun Center in Chester, NY on Friday, July 24, 2015. I am aware that my child will be transported via bus from Myron Urbanski Park to The Castle Fun Center and back to Myron Urbanski Park. Transportation and 2 hours of non-stop fun are included in the total price of $22.00 per child. We will return to Myron Urbanski Park at approximately 12:30 and the children will eat lunch at camp upon their return.</p><p>Participation Permission and Release In consideration of the Town of Goshen, Village of Goshen and Goshen Joint Recreation Commission granting continuing use of its facilities (or its rented facilities), its program and personnel, I authorize my child to participate in the field trip to The Castle Fun Center from Summer Day Camp. I hereby release them from any liability, claims, damage or expense sustained by my child in connection with such participation. In case of injury I give permission for my child to be taken to the hospital for any needed care. I understand the Camp Director will attempt to contact me at the number(s) I’ve listed on this form if an emergency occurs. </p><p>I have discussed with my child that s/he will obey the staff and follow camp rules at all times.
