<p> Name:______Date:______Procedure 43-10</p><p>Assist a Patient To and From an Exam Table</p><p>Objective: The student, using the supplies and equipment listed below, will demonstrate how to transfer a patient from a wheelchair to an examination table and from an examination table to a wheelchair. </p><p>Supplies: gait belt, long-handled stool (if exam table is not equipped with pull-out step)</p><p>Affective Behaviors: Affective behaviors provide a professional approach to a skill that enhances the patient encounter. These behaviors may also display sensitivity to patient’s rights and enhance communication. Pay close attention to these skills which will be in bold, italicized font.</p><p>Notes to the Student:</p><p>______</p><p>Skills Assessment Requirements</p><p>Read and familiarize yourself with the procedure; complete the minimum practice requirements. Document each MPR using proper charting technique. Complete each procedure within a reasonable amount of time, with a minimum of 85% accuracy. </p><p>POINT VALUE  = 3-6 points Frazier/Malone Clinical Medical Assisting  = 7-9 points</p><p>Name:______Date:______</p><p>PRACTICE GRADED GRADED NOTES: TRIAL TRIAL TRIAL # 1 # 2 1.  Greet and identify the patient, introduce yourself and explain what you are going to do. 2.  Wash your hands. 3.  Position the wheelchair so that the patient is sitting with his or her strongest side next to the examination table. 4.  Lock the wheelchair brakes. 5.  Place the gait belt snugly around the patient’s waist, making certain the belt is tight enough that it will not slip and put unnecessary pressure on the ribs. Tuck any excess belt length under the belt. 6.  If the wheelchair allows, remove the foot rests. If not, move them as far out as possible to avoid hitting your shins against them during the transfer. Ensure that the patient is ready prior to beginning the transfer. 7.  Standing directly in front of and as close to the patient as possible, grip the gait belt with both hands in an underhand grip. Bend at the knees and hips to avoid back strain. 8.  If the patient is able, have him or her grip the arm rests and push off at the same time that you lift, for added leverage. If possible, the patient can also assist by pushing upward with his or her legs. Instill confidence by encouraging the patient.</p><p>Frazier/Malone Clinical Medical Assisting</p><p>Name:______Date:______</p><p>9.  With the patient now standing, have him or her place the stronger leg on the stool or exam table step, and together you will lift as the patient steps up. 10.  Have the patient place one hand on the table and guide him or her to a sitting position. 11.  Move the wheelchair out of the way. 12.  To transfer the patient back to the wheelchair: After identifying the patient, explaining the procedure, and washing your hands, place the stool (if the exam table does not have a step) next to the exam table.</p><p>13.  Place the wheelchair next to the exam table with the brakes locked. 14.  With a firm underhand grip on the gait belt, assist the patient to a standing position. If the patient is able, have him or her push off with the legs and arms. Once the patient is steady on the step or stool, have him or her step to the floor with the stronger leg. 15.  Have the patient take small steps backward until the backs of the knees touch the wheelchair. 16.  Ask the patient to reach back and place the hands on the wheelchair armrests for support. Bending at the hips and knees, slowly lower the patient to the chair. 17.  Help the patient adjust to a comfortable position in the wheelchair.
