<p> Steps to filling out required MISD Paperwork On-Line</p><p>1- Go to http://mckinneyisd.rankonesport.com</p><p>2- Click Electronic Participation Forms- You will then see all the required forms to be filled out.</p><p>3- Click on each form individually and make sure that you put your Athlete’s First name and last name, as well as ID Number at the top of each page. (Just put the ID Number, no “S” in front of it)</p><p>4- Be sure that you click the School Attending in 2013-2014 box and select McKinney North.</p><p>** You must do this on every form**</p><p>5- Fill out each form completely and sign where it asks you to (Both Parent and Athlete).. Certain forms with signatures require you to click the “I agree” box. </p><p>6- At the bottom of each page you will see a box that will ask you to put in your e-mail address.</p><p>7- Then click the “Submit” box .</p><p>8- After clicking the “Submit” box if you missed an area it will let you know that by showing what you missed with the words “required” in red.</p><p>9- If all is good you will see a screen that says “You have successfully submitted your form and it has been sent to the appropriate school”</p><p>10- Then click Electronic Participation Forms at the top of the page and move to the next form in line.</p><p>Listed Below are the required forms in order so you can keep track of what you have submitted.</p><p>1- General Information Form</p><p>2- MISD Regulations / Expectations</p><p>3- MISD Information Release Form</p><p>4- UIL Acknowledgment of Rules</p><p>5- MISD Residence Form</p><p>6- Cardiac Awareness Form</p><p>7- UIL Steroid Form</p><p>8- Co-Curricular / Extra-Curricular Contract Steps to filling out required MISD Paperwork On-Line</p><p>9- Random Student Drug Testing</p><p>10- Concussions Guidelines and Consent</p><p>11- MISD Emergency Contact Form</p><p>12- UIL Physical (There is not anything for you to fill out here.
