Tell your doctor about the PAR-Q and which questions you answered YES. -You may be able to do any activity you want — as long as you start slowly and build up gradually. Or, you may need to restrict your activities to those which are safe for you. Talk with your doctor about the kinds of activities you wish to participate in and follow his/her advice.</p><p>Informed Consent and Liaibility Waiver</p><p>By signing below, I confirm that I have answered honestly all of the questions on this document. I understand that there is an element of risk involved with any physical activity and that these sessions are designed in such a way to minimise the risk of injury. However, I am taking part at my own risk and release Joanne Shrewsbury, GI-JO Fitness or anyone associated with GI-JO Fitness from any liaibility for any injuries or damages that may incur whilst participating in the Bootcamp session. If at any time during the session I feel discomfort or pain, I understand that I should notify the instructor immediately. I will make the instructor aware of any illness/injuries that I have and if necessary, will seek advice from my GP to ensure that I am able to participate in the Boot Camp. You are to provide a letter from your GP clearing you for exercise and specifying what, if anything, they say you should not be doing.
