At this time, the only registration needed for Myers Corners students would be for a Physical Appointment if your child needs one.</p><p>**Your child requires a current physical on file dated September 5, 2016 or after. If you do not provide one, your child’s Health Office is scheduling dates to provide school physicals. These dates are posted on FamilyID under your child’s current school. You may sign up for physical appointments starting on March 1, 2017, using FamilyID (click on the link above) in advance of the sports registration. If you are unsure if your child requires an up-to-date physical, please contact your child’s Health Office and the School Nurse will check for you. </p><p>If you need to register your child for a Physical Appointment, click on the following link and follow instructions below: http://www.familyid.com/roy-c-ketcham-high-school-wappingers-junior-high-school/myers- corners-physical-appointments</p><p>1. Click on the green Register Now button. If this is your first time using FamilyID, click on the green Create Account. Click Log In, if you already have a FamilyID account. 2. Create your secure FamilyID account by entering the account owner’s First and Last names (parent/guardian), E-mail address and password. Select I Agree to the FamilyID Terms of Service. Click Create Account. 3. You will receive an email with a link to activate your new account. (If you don’t see the email, check your E-mail filters (spam, junk, etc.). 4. Click on the link in your activation E-mail, which will log you in to FamilyID.com 5. Once in the registration form, complete the information requested. All fields with a red asterisk are required. 6. Click the Save & Continue button when your form is complete. 7. Review your registration summary. 8. Click the green Submit button. After selecting ‘Submit’, the registration will be complete. You will receive a completion email from FamilyID confirming your registration. 9. At any time, you may log in at www.familyid.com to update your information and to check your registration(s). To view a completed registration, select the 'Registration' tab on the blue bar.
