Answer No. 2: Page 24, Entitled, “Scope of Work and Specific Conditions Response” complete Market Basket that applies to product you are offering. Question No. 3 There is a recently added notation stating that no exceptions can be made to the MSA agreement. Please explain how this affects the deviations scope section. REQUEST FOR PROPOSAL (RFP) ADDENDUM NO. 5 Supplemental Curriculum Materials Project No. 16-02-33-A</p><p>Answer No. 3 Part A- “No Exceptions” will be made to MSA. Part B- “Scope Section applies to the market basket you are submitting for under this section.”</p><p>Question No. 4 Submission of proposals: Economic price adjustment. Are we able to state on the exception form that while the % discount will remain the same, the manufacturers pricing (and therefore, our pricing) are expected to increase by 3-8% on January 1st? How are we able to make amendments to Form F to account for this?</p><p>Answer 4: Yes Part B: suppliers will be allowed quarterly to submit any manufacturer price increase for considerations.</p><p>Question No. 5 Freight Terms. We are unfamiliar with the terms 'Prepaid and Allowed' mentioned in your section discussing freight. Please explain what they mean.</p><p>Answer No. 5 FOB destination, freight prepaid and allowed. The seller pays and bears the freight charges and owns the goods while they are in transit. Title passes at the buyer's location.</p><p>Question No. 6 Please explain how Periodic Preformatted Flat File updates work. We've had trouble in the past getting help when we try to update.</p><p>Answer No. 6 “Form F” will be uploaded my Procurement IT Team. Instructional assistance is available for updates. </p><p>This addendum must be returned with your sealed RFP.
