Hospitalizations, amputations or the loss of an eye must be reported within (24) hours. *Drug and Alcohol tests are required for drivers when: A) a fatality has occurred or (317) 232-2693 Monday-Friday 8 am – 4:30 pm. You must only report to OSHA each inpatient hospitalization that involves care or treatment.</p>
