<p>STUDENT PROJECTS: Student Request for IT Support</p><p>Student Group: Class: Applied Semester: Advisor or Faculty Member: UVU Dept. or Client Involved: Project Sponsor: Your group will need to nominate a project sponsor who is a long-term contact at UVU. This could be the advisor/faculty member, someone from the UVU department involved, or someone from UVU IT. The project sponsor must agree to be responsible for a general knowledge of the project and its progression, and to act as a permanent back-up contact in the event that the student group becomes uninvolved before its completion.</p><p>Project Title: Project Description: Project Background: What inspired this project? Was it on the suggested project list? Will it solve a specific issue or improve an existing process? Remember, IT can only support projects that provide some benefit to UVU or its departments.</p><p>End Goal for Production: Upon successful completion, how will the final product be used? What does your group need from IT in order to be successful? Based on your knowledge at this stage, describe the kind of support, tools, and/or equipment you think your group will need to successfully deploy your project.</p><p>PROJECT CONTACT LIST NAME PHONE EMAIL STUDENT GROUP</p><p>ADVISOR/FACULTY MEMBER</p><p>UVU DEPT. OR CLIENT</p><p>PROJECT SPONSOR</p><p>After your group completes this form, attach it in an email to Tom Rasmussen at [email protected]. Type ‘Student Project’ in the subject line, and let him know you are interested in receiving IT support for your project. </p><p>Tom or someone from the Projects Office will get back with you to clarify any information and to set up an initial meeting with IT Committees. </p>
