<p> KAPPA KAPPA KAPPA Alpha Chapter, Bloomington, Indiana 2013 Scholarship Application (BHSN & BHSS) DEADLINE: April 15, 2016</p><p>INFORMATION AND INSTRUCTIONS</p><p>Kappa Kappa Kappa, Alpha Chapter, annually awards two $1,000 scholarships to a graduating senior from Bloomington High School North and Bloomington High School South (one scholarship at each school). Scholarships may be used at any approved and accredited college or university in the State of Indiana.</p><p>Scholarships must be used the year immediately following high school graduation. Payment is made in two installments: half at fall semester enrollment and the other half at second semester enrollment provided the recipient has maintained a “C” average.</p><p>Applicants must plan to remain enrolled at a college or university in the State of Indiana for the next academic year (or two consecutive semesters) to maintain eligibility for this scholarship. The scholarship is not transferable to an out-of-state college or university. Transferring to an out-of-state program during the award period will make the recipient ineligible for the full or any remaining portion of the scholarship.</p><p>Scholastic ability, academic potential, personal statement, financial need, and extracurricular activities are the criteria used to select recipients. The members of the Alpha Chapter Scholarship Committee will review all applications and determine the scholarship recipients. Applicants will not be interviewed. The recipients will be announced no later than April 30, 2016.</p><p>CHECKLIST FOR A COMPLETE APPLICATION</p><p> Completed application form (two pages). NOTE: An electronic version of the application is on file at the BHSN and BHSS Counseling Offices.</p><p> Official high school transcript. If your Social Security Number is printed on the transcript, please black it out.</p><p> A minimum of two (2) letters of recommendation in support of the applicant’s credentials. When seeking a letter of reference:  Be sure to give ample time so your reference can adequately support your application. Ask for letters of support now. Don’t wait until the last minute.  Consider how long and how well you have known the reference. For example, an occasional meeting with a counselor or a random meeting with an administrator doesn’t necessarily mean that the individual will feel comfortable serving as a reference.  Remember, letters of reference fulfill a specific function: to provide an honest evaluation of an individual. Try to identify someone who can provide an honest assessment of your abilities. Letters should include at least one (1) recommendation from each of the following groups: 1. Teacher, faculty advisor, school administrator, or guidance counselor (ex. teacher from a favorite class, school staff members who may know you from your involvement in school activities, a teacher you may work with outside of classroom activities) 2. Employer or extracurricular activity advisor (ex. coach, music/dance instructor, staff from volunteer organizations), or a personal reference from someone who knows you well (preferably not a relative)</p><p>You will need to submit the original documents, and one set of copies. All items should be submitted in the order listed above. Please staple or clip items together and place them inside a folder or large envelope.
