<p> GM-7.3 Children’s Integrated Services Guidance Manual Chapter 7- One Plan Updated as of: June 2016</p><p>Child Care Provider/Program Client Paperwork Guidance</p><p>1 Process Documentation</p><p>Phone call or in-person contact is made. Referral Referral sources include: CIS 01 (referral) Provider (either teacher or director) EEE Parent CIS Provider Licenser Community Agency staff</p><p>Issues typically include:</p><p>Program-related issues (whole program or classroom-specific) Child-related issues (either group or individual) Incident focused (ex. licensing visit or biting)</p><p>Warm call to the provider (note: if child- Initial Contact & Intake specific issue, need parent permission CIS 02 (intake) and 03 (authorization) to proceed. However, you can still proceed with programmatic issues without parental consent). Information gathered:</p><p>Contact info. Initial Contact date Discussion re: issue</p><p>Initial site visit to the program. Intent is One Plan: “Your Resources and Supports”; to gather more information about the “Evaluation Consent” need. May be scheduled with the teacher or director. Visit may include:</p><p>Observations Interviewing staff/director Consultation Meeting with staff to talk with them about what consultation can offer</p><p>How you proceed with this visit is determined by:</p><p> how much information you gathered 2 during the warm call the content of the information you gathered during the warm call</p>
