<p> LEE COUNTY DEPARTMENT OF SOCIAL SERVICES POSITION ANNOUNCEMENT</p><p>POSTED: Inside/Outside Lee County Dept. of Social Services (7-7-2017)</p><p>POSITION: Social Worker II – Adult Services CAP</p><p>DATE AVAILABLE: Immediately</p><p>SALARY/GRADE: Grade 67 - $39,712.00 STARTING SALARY Range: $39,712.00-63,142.00</p><p>DUTIES/RESPONSIBILITIES: This position is in the Long-Term Care Screening Program with the Adult Services Unit. Assesses nursing home candidates physically, socially and economically to determine if they can be maintained in their own homes at a cost lower than the nursing home cost. Service will include screening, case management, arranging a battery of in-home services and completing the necessary paperwork to assure reimbursement for services rendered. A proficient knowledge of computers is required as Adult Services staff uses a software case management program (CAREWORKS) to store case documentation and transmit claims to Medicaid producing revenues to finance programs. Staff also uses e-CAP, a state-wide software program, to assess client eligibility and needs. Some general intake duties as well as an emergency on call rotation is required.</p><p>KNOWLEDGE/ SKILLS/ABILITIES: Must be able to conduct home visits. The screenings will address physical, mental, emotional, economic and social functions. Responsibilities include working with supervisor in obtaining approval for each plan, arranging for services, monitoring service plan as necessary; counsel recipients and family members regarding long term care services choices.</p><p>MINIMUM EDUCATION/EXPERIENCE: Bachelor’s degree from an accredited school of social work; OR four year degree in a human service field or related curriculum, including at least 15 semester hours in courses related to social work or counseling experience and one year exp. in social work or counseling; OR graduation from a four year college and two years experience in rehabiliation counseling, pastoral counseling, or a related human service field, providing experience in the techniques of casework, group work, or community organization; or an equivalent combination of training and experience.
