<p>INSTRUCTIONS FOR SECURITY CLEARANCE FORMS</p><p>You have received your offer letter outlining the security clearance required for your position. You must complete and submit the security clearance forms with your HR package.</p><p>All new hires must complete the:  “Personnel Screening Consent and Authorization Form” </p><p>You will then select one of the following forms based on your offer letter:  Site Access Security Clearance (SASC) Questionnaire; or  Security Clearance Form TBS/SCT 330-60E</p><p>Step 1</p><p>Please open and save the applicable forms to your desktop following these mandatory steps:</p><p>1. Click on the form. 2. Select open (form will open in a “Read-Only” version). 3. If you receive a “Security Warning” select Enable Macros. 4. Enter some information into the form and then save the form to your desktop.  Please Note: the form will not save if you haven’t entered information in to it.</p><p>Step 2 Each form includes instructions for completing the form (either located prior to the form or at the end of the form). Please ensure you review the instructions prior to entering information into the form.</p><p>Step 3 When you have completed entering your information into the form, you must activate the red “Data Validation” button to validate for missing information. </p><p>Missing information will be highlighted in RED. Please enter the missing information if required, and then active the red “Data Validation” button again. </p><p>If nothing is highlighted, you are finished with the form. Save the form.</p><p>Step 4 Completed forms can be submitted electronically as a PDF or word document via email to [email protected] </p><p>Please Note: Forms submitted incomplete will result in delays to your security clearance.</p><p>If you experience difficulties with the form; or if you have questions pertaining to the security clearance; please contact Personnel Security Services at 1-613-584-3311 extension 44870, 44468, 44961 or 44403.</p>
