Conc.</p><p>Reporting Format 62-550.730(4)(c) Interim Page 2 of 2 Effective Date: Pending INSTRUCTIONS FOR FORMAT 550.730(4)(c) GENERAL This format is to be used when reporting the results of water quality parameter (WQP) monitoring required under 40 CFR part 141.87 (1995). </p><p>WATER SYSTEM INFORMATION. List the Population Interval and the minimum number of sample sites that were reported in the sampling plan in Form 62-555.900(12). Indicate the month and year of the six-month monitoring period. When reporting results for the two initial monitoring periods, indicate whether the reported results are for the 1st or 2nd sampling round. </p><p>TREATMENT INFORMATION. List (check) the corrosion control treatment method used, if any. List any corrosion control additives used by chemical name and brand name and indicate if the additives used have been certified under ANSI/NSF Standard 60 for potable water use.</p><p>STATE DESIGNATED WATER QUALITY PARAMETER VALUES. If the system has WQP values designated by the State then list them, otherwise skip this section.</p><p>MONITORING RESULTS. Use the same WQP identification numbers as those in the approved sampling plan along with the site address. Report results for Conductivity and Temperature during the first two initial monitoring periods only. From entry point samples, report the dosage rate for alkalinity as the dosage rate for any chemical that is added to adjust alkalinity. Report a dosage and concentration for orthophosphate and silicates only if they are used as an inhibitor. Dosage rates need only be reported at the point of entry to the distribution samples and not in distribution samples.</p><p>Reporting Format 62-550.730(4)(c) Interim Page 3 of 2 Effective Date: Pending</p>
