Passport  Must be signed, not be frayed, torn, separating, or altered in any other way  Must be valid 6 months beyond intended stay in China with at least 1 blank visa page (not including the last 3 pages which is for amendment/endorsement only)</p><p>2. If you do not have blank visa pages left on your passport, you will need to renew your passport. You can no longer add visa pages to your existing passport. </p><p>3. Please check your passport’s expiration date. Your passport must be valid 6 months beyond your trip to China. If you need to renew your passport, please go to US Department of State website for more information: http://travel.state.gov/content/passports/english/passports/renew.html </p><p>4. If you do not have a passport, a personal appearance at a Passport Application Acceptance Facility is required, please go to: http://travel.state.gov/content/passports/english/passports/apply.html for more information.</p><p>5. If you have a non-US passport, all of your documentation (Green card, work visa, I-90) must be in order and valid for travel outside of the US.</p><p>6. For any assistance with any of the above, we have a courier service that can expedite and hand deliver documents to the US Passport Office, in the Los Angeles Federal Building. You can contact our office with regards to the fees. </p><p>Visa Application Check List:</p><p>Collections dates by the EMBA Program Office for the group passport/visa collection:</p><p>. EMBA LA Class: Friday & Saturday, August 26th & 27th . EMBA SD Class: Friday & Saturday, September 2nd & 3rd </p><p>1. You need to submit your actual passport as the visa will be attached to it.</p><p>2. You need to complete the China visa application form. Handwritten application WILL NOT be accepted.</p><p>3. You also need to submit 1 colored passport type photo taken within the last 6 months (No party photos – cut to size, no black/white photos) & a copy of CA ID or DL or USC ID</p><p>4. For Non-US passport holders, you need to submit proof of immigration status (green card or Form I- 20).</p><p>5. All applicants must submit a copy of flight information (No need to submit if reservations were made through Crown).</p>
