<p> Background Check & Department of Motor Vehicle Review Acknowledgement Form</p><p>This is to acknowledge my understanding that as a condition of consideration as a Coach with Old Dominion Swim League. I am required to successfully complete a Background Check and Department of Motor Vehicle Review. The Background Check and Department of Motor Vehicle check will be done by TC logiQ of Colorado Springs, Colorado and must have an “approved” rating per the Old Dominion Swim League criterion. I understand that I must complete the Background Check and Department of Motor Vehicle Review prior to my employment as a Coach with an Old Dominion Swim League registered team. If either of the above noted checks or reviews are not deemed “approved” by TC logiQ, I understand that I do have the opportunity to appeal the results to Old Dominion Swim League within 7 business days of notification thereof by Old Dominion Swim League.</p><p>I agree to fully and honestly complete all parts of the Background Check and Department of Motor Vehicle Review to the best of my abilities. Any attempt to omit, or disguise the truth will result in denial of consideration for employment or other actions deemed appropriate by Old Dominion Swim League.
