This form may be used by a court services officer or custodian if the child is not in the secretary’s custody and the secretary is not providing services to the child. This form is to be prepared in consultation with the child, if 14 years of age or older, and the parents, when possible. The purpose of the permanency plan is to assure, in so far as is possible, that children have permanency and stability in their living situations, and that the continuity of family relationships is preserved. The safety and well-being of the children is paramount.</p><p>Comments</p><p>Appeals from a reintegration plan are from the court’s approval of the plan, rather than subject to the agency’s administrative review procedures.
