<p> PROGRAM ADMISSION School of Education Requirements</p><p>Complete and submit the reverse side of this form to Office of Field Services</p><p>The following are required by The Pennsylvania Department of Education and the Commonwealth of Pennsylvania for Teacher Certification. First Year:  In summer or fall, complete the physical exam and TB (Mantoux) test through Keeling Health Center (do not do this during the spring semester).  Complete the Speech & Hearing Screenings through the Speech and Hearing Clinic in Keeling Center  Send a copy and keep a copy of your current shot records to the Keeling Health Center. {Required shots include: Tetanus within the last 10 years (Tdap), 2 immunizations of MMR, Series of 3 HepB}  Complete all clearances –Act 33 (Child Abuse check), Act 34 (Criminal Record check), Act 24 and the FBI Clearance (Finger printing) . Submit copies to Office of Field Services. Keep the originals. You will need more copies to obtain entrance to a school building.  Obtain a million dollar liability insurance policy (Check the OFS website for online providers) recommended is the 4 year plan.  Complete any educational courses you take with a grade of a “C” or higher.  Complete and submit passing scores for all PAPA (Pearson Basic Skills tests)or ETS CORE assessment tests or SAT scores if over 1550 Second Year:  Complete the physical exam through Keeling Health Center. (TB test is good for two years) * * *  Complete Education courses with a grade of “C” or higher.  Earn a cumulative grade point average of 3.0 or higher by the end of the second year or before taking block classes.  Obtain/maintain a million dollar liability insurance policy (Check the OFS website for online providers) recommended is the 3 year plan (if you did not purchase the 4 year plan your first year). Third Year: By following the above sequence, the student will be ready to gain admission into the Teacher Certification Program.  Complete a physical exam and TB (Mantoux) test through Keeling Health Center.  Take the required Praxis II or PECT tests for your given major.  Must have a GPA of 3.0 or higher before taking block classes  Obtain/maintain a million dollar liability insurance policy (Check the OFS website for online providers) recommended is the 2 year plan (if you did not purchase the 4 year plan your first year).  Complete the Program admission application (with advisor signature) and submit it to the Office of Field Services.  Complete all Educational courses with a “C” or higher. Fourth Year:  Repeat all clearances –Act 33 (Child Abuse check), Act 34 (Criminal Record check), Act 24 and the FBI Clearance (Finger printing).
