DEADLINE for scholarship applications must be followed. (NO EXCEPTIONS) 2. Late applications will not be considered. 3. Refer to application process below for a list of the supporting documents needed (i.e., reference forms, evidence of GPA, etc.) Incomplete applications will not be considered. 4. If any question does not apply to you in this application please put N/A in the space. 5. Type or print legibly. Illegible applications will not be considered. 6. You will be notified by phone, email, or mail in May regarding the status of your application. 7. If you have any questions about the application, please email Founder and Director, Staci Ortiz-Davis, at: [email protected]</p><p>PURPOSE The Josie Pearl Memorial Scholarship was established in 2013. The mission of the scholarship is to provide financial assistance to individuals enrolled for undergraduate study in colleges and universities. The foundation principally targets students who cannot afford an education or risk graduating with debt. </p><p>FINANCIAL ASSISTANCE is based on financial need, community service, and academic performance. </p><p>SCHOLARSHIP AWARDS The Josie Pearl Scholarship Foundation Committee awards scholarships on the basis of a comprehensive process, including personal interview. Areas that are reviewed by the committee include, but are not limited to the following: Academic Accomplishments, Community Service, References, Personal Essay and Financial Need. The Foundation pays scholarship funds directly to the recipient’s school. The Josie Pearl Scholarships are awarded without regard to race, color, ethnicity, gender or sexual orientation. Scholarships awarded are based upon the availability of funds and additional qualifying criteria.</p><p>CRITERIA  Applicants must be graduates of a California high school.</p><p> Applicants must be completing or have completed high school successfully with a minimum, unweighted GPA of 2.5 on a 4.0 scale.</p><p> Applicants must be accepted as a full time student at a college or university, for the upcoming academic year. If scholarship money is awarded, this is the only program that will receive the funds on behalf of the applicant.</p><p> Applicants must demonstrate a need for financial assistance (via application or essay.)</p><p>1  Applicants must complete and submit a Scholarship Application by email: April 11, 2014 OR postmarked by Wednesday, April 9th, 2014. </p><p> Scholarship recipients’ college will receive the check only after final high school transcripts have been received.</p><p> Finalists must attend a panel interview in order to be considered.</p><p>TIMELINE  Applications are due by email: April 17, 2015 OR postmarked by Wednesday, April 15th, 2014. </p><p> Candidates who are eligible for interviews will be notified by the Scholarship Committee by late April 2014.</p><p> Applicants are notified if awarded a scholarship by June 2014.</p><p>Application Process </p><p>SCHOLARSHIP APPLICANTS MUST PROVIDE:</p><p> Completed application form (essay included). </p><p> High school transcript from their institution. </p><p> Two academic references from a guidance counselor or teacher. </p><p> A letter completed reference from an employer or other community member.</p><p> Proof of acceptance at a college or university for post-secondary studies.</p><p> Demonstrated financial need (via application/essay). </p><p>SCHOLARSHIP AWARDS</p><p> Award notification will be given by mid-June 2014. </p><p> Applicant must provide their student ID, the correct mailing address of their institution and the department where their scholarship check is to be received.</p><p> Award amounts will be distributed to the college after a student’s final grades are received by the Josie Pearl Memorial Scholarship Foundation.</p><p> If the designated recipient fails to meet the criteria upon graduation, the scholarship will be awarded to the runner up. </p><p>Deadline for the application is by email: April 17, 2015 OR postmarked by Wednesday, April 15th, 2015</p><p>Please email OR mail the application and all forms to:</p><p>The Josie Pearl Memorial Scholarship 2 Attn: Staci Ortiz-Davis P.O.
