<p> Casa Roble High School Nook E-Reader Permissions and Acceptable Use Form Introduction The Barnes and Noble NOOK is a convenient, portable reading device. It is the size of a paperback and capable of holding over 1,000 books. The opportunity to use this device is a privilege that we are able to provide to students with the expectation that the student will use extra caution and responsibility. Each NOOK is valued at $206.00, including the accessories. For this reason we require a parental signature before a NOOK may be borrowed by a student. The protection plan is required if a new NOOK must be purchased as a replacement. The estimated value of the Nook and its accessories are listed below: NOOK $86.00 Case $32.50 Power Cable $16.50 Protection plan $68.00 Parent Responsibilities and Permission I am authorizing the borrowing of a NOOK by my child. I understand that it is to be used as a tool for reading and learning and that my child will comply with the SJUSD Acceptable Use Policy which is available online and was provided when I enrolled. I will help ensure the safe and timely return of the NOOK within the loan period of three weeks. I also understand that I am financially responsible for any willful, malicious, or accidental/unintentional damage to the NOOK and/or any of the accessories. I understand that my child may lose future loan privileges of the device if the NOOK is either damaged or not returned in a timely manner.
