<p> Explanation of Reading Fluency Record Chart (This chart is done in Excel Office program and may be done using that format.)</p><p>Oral Reading Rate Chart Fluency Reading Rate Chart-Diagnostic</p><p>Goal: Write the expected outcome that you wish to have your student attain by the end of the school year or IEP review. This should refer the number of WPM (words per minute).</p><p>Date: This is the actual date that the reading sample is taken.</p><p>Reading Sample: In this area, write a short description of the reading sample. Include the name of the book, page number, or anything that you may need to identify that particular source.</p><p>R.L.: This refers to the Reading Level of the sample.</p><p>Types of Errors Omission: When a student omits a word it is considered an error of omission.</p><p>Substitution: Substitution errors occur when the student will substitute a word with another word and still have the sentence make sense. This usually happens with small words, however, sometimes a student will substitute the word “house” for “home”.</p><p>Addition: Anytime a student adds a word or words to the reading sample, it is considered an error of addition.</p><p>Endings: Errors in this category would include omitting the ending or putting a different ending on a word. Misidentification: An error of misidentification is when a student pronounces the word incorrectly or fails to make an attempt to identify the word.</p><p>Total Errors: Record the total number of errors from the previous five (5) columns. If you are using the Excel program to enter this information, this will automatically be done for you.</p><p>Time: This refers to the amount of time that it takes the student to read the sample. Record this as a decimal if using the download excel format. (Example: If it takes the student 3 minutes and 46 seconds, enter the information as 3.75 or you may round to the nearest tenth.)</p><p>WPM: The words per minute (WPM) is calculated by subtracting the number of errors from the total number of words in the selection and dividing by the time that it took the student to read the selection. This will be calculated for you if using the download format.</p>
