<p> Multi-agency Safeguarding Referral Process from Portsmouth Hospital Trust To Hampshire And Portsmouth Adult Services </p><p>Guidelines</p><p>This Guidance has been completed in conjunction with Hampshire Adult Services (HCC), Portsmouth City Council (PCC) and Portsmouth Hospital Trust (PHT) to help develop a consistent approach in responding to safeguarding referrals from Portsmouth Hospital Trust.</p><p>As from 10 December 2012, the Safeguarding Adults referral process from Portsmouth Hospital Trust will be as follows. Please refer to the flow chart.</p><p> PHT will refer all Safeguarding alerts to Portsmouth city council safeguarding team secure inbox.</p><p> PHT off site locations (Community hospitals) include : Totton, IOW, Bognor, Gosport War Memorial and Petersfield. These locations will continue to make any safeguarding referral to their local authority by telephone. All Community hospitals have a link practitioner from the local area office and will follow the safeguarding process.</p><p> PCC Safeguarding team will screen the referrals from PHT.</p><p> PCC will follow their safeguarding process for all Portsmouth service users.</p><p> PCC will forward all alerts of Hampshire service users to a secure shared inbox at HCC Adult services Hospital team (QA).</p><p> The response to the alert will depend on the level of the alleged abuse (1-3) and location of alleged abuse.</p><p>Process for Hampshire Service user</p><p>1) If the alleged abuse (Level 3) occurs in hospital.</p><p> PCC to take the lead and coordinate the safeguarding process.</p><p> PCC to inform/involve HCC and plan a joint investigation.</p><p>Safeguarding referral process from PHT to Adult services 21 Nov 2012 1  HCC to follow review process and support service user and family.</p><p>2) If the alleged abuse (Level 1/2) occurs in hospital.</p><p> PCC to forward to Hants secure email.</p><p> HCC Adult Services Hospital Team to coordinate Safeguarding.</p><p> HCC to feedback the outcome of investigation to PCC.</p><p>3) If the person is in hospital and the alleged abuse took place in the community.</p><p> PCC to forward the alert to Hants secure email. </p><p> HCC Adult Services Hospital Team to inform Community Team.</p><p> HCC community team to coordinate the Safeguarding process.</p><p>HCC and PCC to Feedback outcomes of the safeguarding investigations to Portsmouth Hospital Trust.</p><p>Safeguarding referral process from PHT to Adult services 21 Nov 2012 2</p>
