<p> Prescription Rx Disclosure</p><p>You are entitled to a copy of your glasses and contact lens prescription. You also have the right to choose where you purchase your glasses and/or contact lenses. A copy will be provided for you upon request. </p><p>We do, however, want you to understand our Prescription Guarantee Policy. When you purchase glasses and contacts from us, we have the ability to do more for you. We can remake a prescription, change a prescription, warranty frames and lenses, and offer trial contact lenses when necessary, all at no extra charge to you. </p><p>When you purchase your frames, lenses and/or contacts lenses anywhere other than our office (Family Vision Care), we cannot warranty the prescription. Any mistakes made by the lab where you purchased the product will not be our responsibility, and no reimbursement will be offered for any extra charges that may incur. </p><p>We also will not be able to recheck the prescription at no extra charge. A minimum of $35.00 will be charged to recheck you again if and when necessary. Other charges may also apply. These are all charges that are waived when you purchase these products from our office.</p><p>We do ask that you give us a chance to match any price you may have found elsewhere. We do offer our contact lenses at the lowest possible price that we are allowed to charge. We also have glasses (frame and lenses) to meet most any budget. We have a selection of FREE frames when you purchase the lenses from us. </p><p>Please do not assume our prices are higher because we are a professional private practice. Give us a chance and check with us first. We want to offer full service to ALL of our patients.
