Purpose: To set forth the guidelines for sending assigned personnel to sick call or to Evans Army Community Hospital Emergency Room for medical treatment.</p><p>2. Responsibilities:</p><p> a. The Commander will ensure overall compliance with the procedures set below. </p><p> b. Soldiers: </p><p>(1) Must inform their immediate supervisor before going to Sick Call. </p><p>(2) Must provide the Company Orderly Room with a copy of the DD Form 689 (Individual Slick Slip) and profile slip after returning from sick call. (Both DD Form 689 and/or Profile will be given by the Physician).</p><p>3. Procedures: All Soldiers must comply with the hospital’s sick call policy and procedures and report any concerns about treatment to the chain of command immediately.</p><p>[Original Signed] MICHAEL B.
