<p> Field Trips</p><p>Teachers are to discuss their field trips with their supervisor and get approval prior to submitting forms to SELPA office.</p><p>Field Trip Process and Forms</p><p> “Throughout the School Year” is for community training that you will be doing with your students throughout the school year that is part of their IEP or your classroom program. These trips are to take place within the city your SDC is located. This form is for individual or small group field trips only. </p><p> o You will need a Field Trip Report submitted to SELPA office that details the sites you will be visiting throughout the year and the days of the week and/or times.</p><p> o You will then need two “Throughout the School Year” authorization forms signed by the parent of each child. One for your file and one for the SELPA office file.</p><p> o An adult in the classroom is to know where the students off campus are at all times so that the students can be located in case of an emergency. </p><p> Whole Class Field Trips</p><p> o A Field Trip form must be completed one month prior to the date of the trip, so teachers will need to plan ahead for these outings.</p><p>. All whole class trips within the city, county, or to Oregon</p><p>. SELPA office will notify SDC Teacher when field trip is approved and then forms to parents can be sent home.</p><p> o Parents are to sign two “Single Event” authorization forms</p><p>. One form is kept by the teacher to be taken with them on the day of the field trip because no one will be left in the classroom.</p><p>. One form is kept by the SELPA office</p><p> o SELPA office is to be emailed or called with the students who are going on the day of the field trip.</p><p> General Education Field Trips</p><p> o The student will require district general education forms completed and submitted to the general education teacher</p><p> o SDC teacher is to complete a SCOE Field Trip Report for the activity also</p><p> o The student will also require two signed “Single Event” authorization forms </p><p>. One form is kept by the SDC Teacher . One form is kept by the SELPA office</p><p> o SELPA office is to be emailed or called with the students who are going on the day of the field trip.</p><p> Adults attending Field Trips</p><p> o If the adult is an employee of SCOE no forms are needed</p><p> o If the adult is not an employee they will need to complete two signed adult authorization forms.</p><p>. One form is kept by the SDC Teacher </p><p>. One form is kept by the SELPA office</p>
