<p>Dear Interested Company/Party: </p><p>Bechtel National, Inc. will be providing design, procurement, construction, startup, and operational services for the Pueblo Chemical Agent Disposal Pilot Plant (PCAPP) Project. We are currently in the process of developing preliminary bidder’s lists.</p><p>We are extending an invitation to your firm to submit a pre-qualification package to us. In order to be eligible for prequalification consideration you must complete the following:</p><p>1. Contractors Questionnaire, 2. ES&H Questionnaire 3. Environmental Questionnaire</p><p>All questionnaire’s must be filled out in their entirety and provide all required documentation spelled out in the aforementioned. If necessary please advise the individual below that you will be using your Interstate Experience Modification Rate. Provide the release form (attached to this package) to NCCI in order for Bechtel to confirm this Interstate EMR.</p><p>As a minimum, your company must meet the following hurdle rates for ES&H eligibility in order to be considered for prequalification:</p><p>EMR 1.0 or lower IR 7.5 or lower LTIR 4.0 or lower</p><p>IR is defined as recordable Incident Rate, LTIR is defined as lost time incident rate. The Incident Rate is the number of injuries, illnesses, or lost workday cases related to a common exposure base enables one to make accurate industry comparisons, trend analysis, overtime, or comparisons among firms regardless of size.</p><p>If your company has not had a DUNS number for a minimum of 2 years, please submit 3 years of audited financial statements for commercial evaluation. If sufficient information is not available from the DUNS number, Bechtel reserves the right to request additional financial information for evaluation from an otherwise qualified firm.</p><p>Please be advised that being prequalified does not automatically guarantee technical acceptance during the proposal phase should you be invited to bid on this scope of work. Should you fail to qualify you will be notified by letter with a brief explanation of the deficiency.</p><p>In addition to these forms, please provide an uncontrolled sample of your firms Corporate Environmental, Safety and Health Plan, Quality Assurance plan. Please feel free to supplement this package with brochures and other information that documents your firm’s, capabilities, experience, etc.</p><p>The work on this project will be governed by the National Construction Agreement and local addendum. All subcontractors will be required to sign and abide by this agreement. In addition it should be noted that should you be chosen to submit a proposal for this scope of work, a proposal bond will be required in the amount of 10% of the total proposal price and should you further be awarded this scope of work a payment and performance bond in the amount of 100% of the proposed value as set forth in the proposal. </p><p>Please identify your package with the notation “Prequalification Submittal for PCAPP Project Subcontract Name” on the outer envelope. Preferred receipt of this package is by return email, followed by US Mail or Federal Express.
