Up to FIVE wells may be covered by one permit (only one fee is charged).</p><p>A complete application consists of:</p><p>1. This completed application form. We must have original signatures for our records. Include an electronic copy of the completed application materials either via email or include a disk. 2. The $1,000 application fee. 3. A $10,000 bond or security, per hole. 4. Evidence of mineral rights ownership or lease.</p><p>APPLICATION FORM The application is generally self-explanatory.
