<p> Deva Health, PLLC</p><p>Practice Guidelines</p><p>Welcome to Deva Health, PLLC!</p><p>Because this is not a regular primary care practice, we have a few guidelines for patients.</p><p>** Primary Care Physician—We require that you be an established patient with a primary care physician near your home. Dr. Barry works with your regular doctor but does not replace them for day- to- day needs that may arise after hours or on weekends. We try to respond to needs as they arise related to your care with us, but our practice is not equipped to deal with emergencies.</p><p>** Treatment Plan—Dr. Barry will devise a treatment plan in consultation with you, but you must agree to follow Dr. Barry’s recommendations closely. You must notify the practice if you are unwilling or unable to follow the agreed upon treatment plan. IF YOU ARE PRESCRIBED ANTIBIOTICS YOU MUST FOLLOW THE PLAN EXACTLY AS WRITTEN, or we may be required to release you from our practice.</p><p>** Release from our Practice—If it is determined that you are not compliant with the treatment plan or issues arise that interfere with your effective treatment, we may release you from our practice and you will have to find treatment through a different provider.</p><p>** Questions—We encourage you to contact the practice with questions that arise, particularly during your first 90 days. The disease processes treated in our practice can be quite complex and questions come up from time to time.</p><p>** Emergencies—If you think you are experiencing a medical emergency, please call 911. If you have a semi-urgent issue that needs attention, you may contact us by phone (do not use email) but if you do not hear back from us promptly, seek medical attention elsewhere (this is one reason that we require you have an active relationship with a PCP!).
