<p> Required Documentation for the Early Intervention Program Paraprofessionals, Teacher Assistants and UWF mentors</p><p>1. Small Group Instruction Monthly Profile Sheet for students receiving Tyner instruction</p><p> Complete monthly (as close to end of month as possible)</p><p> Complete the chart using the word study level and book level that you are working on in group with the child. If you will keep the chart with your plans, then you can quickly fill it out and keep it current.</p><p> Make a copy and turn completed chart in to the AIS the following week . </p><p>2. Attendance</p><p> Attendance should be kept in an attendance book for the purposes of documentation. This intervention cannot be expected to work if the student doesn’t receive it as designed, on a daily basis. If they are not present in the group, it needs to be noted.
