Box 13582 RTP, NC 27709</p><p>Re: Approval for Evaluation of HPS-5 Polyurea Pavement Marking </p><p>Dear Mr. McSwain:</p><p>This letter is to approve the use of HPS-5 on a limited basis for application on construction contracts for the purpose of evaluating the performance of this material for Polyurea use on Minnesota highways and for inclusion on the approved products list for Mn/DOT Polyurea pavement markings. This approval is valid until August 15, 2009 and is limited to a total of 3500 gallons of material applied on state projects conditional on meeting all conditions contained in this letter.</p><p>The conditions for application on state projects are:</p><p>1. All requirements of the Pavement Marking Qualification and Acceptance Program, found at: http://www.dot.state.mn.us/trafficeng/products/2004PavementMarkingApprovalProgr am.pdf, are satisfied. This included the need to supply a technical contact name, number, e-mail address.</p><p>2. Need to submit a sample of the product (including Quality Control Test results, shipping information, and product certification) to the Mn/DOT Materials lab at:</p><p>Jim McGraw Office of Materials and Road Research 1400 Gervais Avenue Maplewood, MN 55109 Phone: 651-366-5548</p><p>3. Project Engineer approves of the substitution of material on each project.</p><p>4. The vendor will contact the Pavement Marking Engineer 48 hours prior to placement of the product.</p><p>5. The vendor and/or contractor will complete the attached Contractor Striping Report Form and submit this information within 5 working days of installation to Pavement Marking Engineer, at fax 651-234-7370.</p><p>An equal opportunity employer 6. The contractor and manufacturer shall discuss the details of the Polyurea application as it will be done below the temperatures as specified in the original product specifications.
