<p> BLUEBONNET TRAILS COMMUNITY SERVICES</p><p>Original effective date: 4/14/03 Operating Procedure Revised: HIPAA Approved: Disposal of PHI </p><p>DISPOSAL OF PROTECTED HEALTH INFORMATION</p><p>PROCEDURE: Bluebonnet Trails Community Services (BTCS) will dispose of protected health information only by means that it assures it will not be accidentally disclosed to an outside party. BTCS will ensure the appropriate means of disposal are reasonably available and operational. </p><p>The BTCS workforce will strictly observe the standards related to disposal of hard copies and electronic copies of protected health information.  BTCS will not discard protected health information in general trash bins, unsecured recycle bags or in other publicly accessible locations.  As the method is reasonably available and operational, BTCS will shred, incinerate or place in secured recycling bags the protected health information to be disposed of in a manner that ensures confidentiality.</p><p>Hard Copy Destruction. Secure methods are used to dispose of hard copy data and output. BTCS Medical Records Department is responsible for overseeing the destruction of hard copies of protected health information.  Printed protected health information will be shredded by and/or recycled by a firm specializing in the disposal of confidential information or shredded by an employee, volunteer or trainee authorized to handle and personally shred the protected health information. If this option is unavailable, BTCS will incinerated the confidential information using an employee or firm authorized to destroy the protected health information. Microfilm or microfiche will be cut into pieces, chemically destroyed or incinerated </p><p>Electronic Copy Destruction. Secure methods are used to dispose of electronic data and output. The BTCS Information Services Department is responsible for the destruction of electronic media containing protected health information. This media includes but is not limited to computer hard drives, floppy disks and compact disks. Proper disposal of electronic media is managed by using the following methods:  deleting online data using the appropriate utilities;  removing protected health information from mainframe disk drives on computers or other electronic devices that are being replaced or removed;  erasing or wiping clean diskettes to be reused by using the appropriate utility to prevent recovery of data designed to eliminate ghost images; or  physically destroying discarded diskettes. Workforce: Employees, volunteers, trainees and other persons whose conduct, in the performance of work for BTCS, is under the direct control or supervision of BTCS whether or not they are paid by BTCS.</p>
