To join us, you will need the following information:</p><p> Age Verification Document (ie., birth certificate; religious, hospital, or physician’s document showing date of birth; adoption record; affidavit from parent; previously verified school records, etc.)</p><p> Proof of residency for the Jackson School district (Copy of utility bill, lease agreement, etc., showing the physical address)</p><p> Copy of student’s social security card. Providing this information is strictly voluntary. Your Social Security number is used for federal funding purposes.</p><p> If available, recent photograph (school picture type)</p><p> Unofficial copy of transcript to ensure correct scheduling/placement & withdrawal form from previous school. (We will request official records after enrollment).</p><p> Proof of legal guardianship if you do not live with your parents or if your parent’s name is not on birth certificate. A copy of divorce decree is needed so JHS can follow court guidelines and protect students. Temporary guardianship must be settled through a lawyer.</p><p> Valid Alabama Immunization Form (“Blue Card”) & Alabama’s State Health Form </p><p> Completed Enrollment Form</p><p> Address and phone number of previous school</p><p> Home Language Survey/Technology Agreement signed by student & guardian.</p><p> Additional information to help us serve you to the best of our ability. </p><p>The Clarke County Board of Education does not discriminate on the basis of race, color, national origin, sex, disability, or age in its programs and activities and provides equal access to the Boy Scouts and other designated youth groups.
