Please initial in space provided after reading each point.</p><p> I acknowledge that use of the laptop is governed by the Library’s Internet Safety Policy and Code of Conduct and I agree to comply with these.  I have read & agree to the terms of the Laptop Policy.  I understand that this equipment is on 2-hour library loan and cannot leave the library.  I will inspect the laptop upon checking it out to confirm that it is in good condition.  I will not tamper with the hardware or software.  I agree to protect this equipment from theft and/or damage.  I will not leave the laptop unattended.  I will not have food or drink in the work area when using the laptop.  I acknowledge that the Library is not responsible for damage to files or removable media.  I understand that the Library is not responsible for lost files and I am responsible for saving my work on a device (e.g.. flash drive) of my own.  I agree to return the laptop to the Circulation Desk by the time due and personally give it to a library staff member. I will return the laptop no later than 30 minutes before the Library closes.</p><p>Laptop privileges may be revoked if a borrower violates any part of this agreement. </p><p>My signature below indicates that I have read the Laptop Liability Form.
