<p> ALLIANCE FOR WOMEN & CHILDREN FACLITIES RENTAL CONTRACT NONPROFIT & SCHOOLS</p><p>Alliance for Women & Children building is available for rental: Monday through Friday 8:30 a.m. to 5 p.m. Other times may be discussed as needed</p><p>General Requirements</p><p>1. The lessor (Alliance for Women & Children) reserves the right to refuse rental. 2. A $100 deposit is required at signing of contract. Failure to meet contract expectations may result in a partial or total forfeiture of the deposit. This applies to rental of the playground and imagination playground as well as other Alliance facilities. A key may be provided to the renter for necessary access. If the key is not returned to the Alliance for Women and Children within 5 business days of the rental date a $25 fee will be assessed and taken from the deposit. The balance of the deposit less the rental fee will be returned to the renter within 5 business days of return of the key. 3. Rental date will not be reserved for lessee until the fee is paid and signed contract is returned. 4. Lessee may schedule time in advance of the event to set-up. Set up must be pre-arranged and during normal business hours. 5. Lessee is responsible for leaving the facility clean and in same condition in which it was found. Table and chairs must be wiped clean and restored to their original positions. Trash must be put into furnished trash receptacles. If the kitchen is used, all counter tops must be wiped and sinks must be free of food particles. A minimum $50 cleaning fee will be assessed if the room is not left in its original condition. If playground is used, gate must be closed. If imagination playground equipment is requested, participating adults must have completed background check and training date on file. Container may not be moved or repositioned. Container must be checked for damage, inventoried and locked. Additional provisions apply. 6. The possession of or the serving of alcohol is not permitted at any time. 7. Lessee is responsible for any damage to equipment and / or building. 8. Smoking is not permitted in the building. 9. No tape, nails or tacks may be used on the walls or ceilings. 10. Compliance with all city, state and federal laws is required. 11. If use of facility is scheduled for an evening event, lessee is required to arrive at the Alliance for Women & Children office by 4:30 p.m. to make arrangements with a staff member regarding secured access of facility, use of equipment, etc. ____Please initial. 12. Alliance for Women & Children accepts no liability against any and all loss, cost (including statutory liability and liability under worker’s compensation laws) in connection with claims for damages as a result of injury or death to any person or damage to any property sustained by the individual(s) using the facility or any and all other persons which arise from, or in any manner grow out of, any act or neglect on or about the facility by the individuals using the facility, guests or invitees. ______Please initial.</p><p>______Lessee Signature Date </p><p>______Alliance for Women & Children Representative Signature Date Room Accommodations:</p><p>_____ Board Room (1,130 sq. ft)</p><p> Seats 30-40 people theater style (without tables) Seats 25-30 with tables Rental fee during normal business hours - $ Free. If special room set-up is required there will be a $75 set-up fee. Rental fee for evenings and weekends - $75 for the first 2 hours and $25 for each hour thereafter. </p><p>_____ Auditorium (1,802 sq. ft)</p><p> Seats 120-150 people theater style (without tables) Seats 80-100 with tables (limited number of chairs available) Rental fee during normal business hours - $Free. If special room set-up is required there will be a $75 set-up fee. Rental fee for evenings and weekends - $125 for the first 2 hours and $35 for each hour thereafter. </p><p>_____ Kitchen</p><p>If lessee chooses to use the kitchen during the rental for a served meal, there is an additional cost of $75. It is equipped with 2 ovens and stovetops, sink and large counter work area. The kitchen is adjacent to the auditorium. We require the kitchen be thoroughly cleaned by the rental group following use.</p><p>_____ Classroom(s) (please define)</p><p> Seats 20 people theater style (without tables) Seats 12 with tables Rental fee during normal business hours - $Free. If special room set-up is required there will be a $50 set-up fee. Rental fee for evenings and weekends - $50 for the first 2 hours and $25 for each hour thereafter. </p><p>_____ Playground</p><p> Accommodates 30-40 children at play. Equipment is designed for children 12 and under. Includes a play structure with slides and climbing apparatuses, a stage, benches, etc. Rental fee during normal business hours - Free. Rental fee for evenings and weekends - Free. Must be reserved for purposes meeting non-profit/school mission. Not for personal use. Rental fee during normal business hours does not ensure a private party</p><p>_____ Imagination Playground (INDOOR (see Auditorium) ______or OUTDOOR ______)</p><p> Accommodates 30-40 children at play, working together. Equipment is designed for children over 2 years old. Rental fee during normal business hours - Free. Rental fee for evenings and weekends - Free. Must be reserved for purposes meeting non-profit/school mission. Not for personal use. Auditorium rental fees will be assessed for indoor Imagination Playground rental. Rental fee during normal business hours does not ensure a private party</p><p>LESSEE IS TO PROVIDE: Paper goods, food and drink supplies, linens, toys and play equipment.
