State Assessment Incident Reporting Procedures

<p> 2011-12 Georgetown ISD State Assessment Incident Reporting Procedures (including Student Cheating)</p><p>1. Immediately report all potential incidents/testing irregularities/confirmed student cheating to the GISD Assessment Director (Becky McCoy, ext. 6034) or Secretary (Denise Gunn, ext. 6098) and to your campus principal. Assessment staff will complete an Incident Report Form to log the incident and assign a tracking number.</p><p>2. The DTC will review the incident and determine if a testing irregularity has occurred. If no irregularity occurred, the Assessment Dept. will email the Campus Test Coordinator and principal the completed Incident Report Form with confirmation that no further action is required. </p><p>3. If a procedural testing irregularity has occurred, the CTC will be required to email to the Assessment Department a description of the incident and measures that can be taken in the future to prevent a similar incident.</p><p>4. If a serious testing irregularity has occurred, the CTC will be required to complete a Testing Irregularity Report and get written/signed statements from all individuals involved. </p><p>5. If student cheating was confirmed by campus personnel, the CTC will be required to email to the Assessment Department a description of what occurred and any disciplinary actions stemming from the cheating.</p><p>6. Submit all documentation to the GISD Assessment Department by the specified deadline.</p><p>7. The DTC will report all incidents to TEA in accordance with required guidelines.</p><p>8.
