<p> FORT IRWIN LEGAL ASSISTANCE OFFICE</p><p>Bldg 230 (C Ave)/ 760-380-5321/ DSN 470-5321 Adjustment of Status Checklist</p><p>To help your qualifying family member become a lawful permanent resident in the United States, you must file the following items with the U.S. Citizenship and Immigration Services:</p><p> All forms are available at the USCIS website- www.uscis.gov or from your attorney.  Form I-130, Petition for Alien Relative, completed and signed by you. OR  If your relative has already been approved for an immigrant petition, you must submit a copy of the approval notice (Form I-797, Notice of Action) sent to you by the USCIS.  Form I-485, Application to Register Permanent Residence or Adjust Status, completed and signed by the person seeking adjustment of status (i.e. your spouse, child, or parent).  Form G-325A, Biographic Data Sheet (two, one each for you and your relative).  Form I-693, Medical Examination Sheet (must be conducted by a physician designated by the USCIS as a Civil Surgeon)  Two passport-style color photos taken within 30 days. (Two each of you and your relative.)  Form I-765, Authorization for Employment (if seeking employment while case is processed).  Evidence of inspection, admission or parole of your relative into the United States (e.g. stamped foreign passport and/or Form I-94, Arrival Departure Record).  Form I-864, Affidavit of Support (completed by the sponsor).  Supporting documents needed for the above forms include, but are not limited to the following: 1. Proof of sponsor’s US citizenship: birth certificate, passport, or certificate of naturalization 2. Relative’s birth certificate from home country, with English translation. 3. Copy of marriage certificate or license, with English translation, if needed. 4. Copies of last three years Federal Income Tax Returns, W-2s, and service member’s LES for the last three months. 5. Letter from service member’s commander stating current duty status, length of time in service, rank and monthly basic pay.  Current Fees (Check or Money Order made payable to Department of Homeland Security) 1. Form I-130, $420.00 2. Form I-485, $1070.00 ($985.00 plus biometric fee of $85.00) 3. Form I-765, $380.00 4. Fees for the Medical Exam vary greatly, so call around to get the best price.</p>
