<p> INTERCONNECTION CUSTOMER’S LETTERHEAD</p><p>AFFIDAVIT</p><p>This affidavit is being submitted in satisfaction of the requirements in Section 25.1.2 of the CAISO Tariff of the California Independent System Operator Corporation (“CAISO”).</p><p>(1) I, [authorized representative’s name], the undersigned, as a representative of the [Interconnection Customer’s name and complete street address], am authorized to execute this affidavit on behalf of [Interconnection Customer].</p><p>(2) [Interconnection Customer] is the legal owner of [Generating Facility’s name] , a __MW [insert technology type] generating facility located at [Generating Facility address] and connected to the ISO Controlled Grid. </p><p>(3) [Interconnection Customer] represents that [Generating Facility’s name], [QF ID / Log. No. / Project ID] is a qualifying facility (“QF”), in accordance with the Public Utility Regulatory Policies Act of 1978 (“PURPA”) and Part 292 of the Federal Energy Regulatory Commission's Regulations (18 C.F.R.
