Thank You for Your Prompt Attention to This Matter

Dear Students,

Please find attached to this e-mail instructions for renewing your CastleBranch account. All students are required to renew your account and complete your Cogent Fingerprint and DHS Child Abuse background checks as directed in the instructions. This must be done by April 16, 2017, in order to be able to start your upcoming IPPE or APPE rotations.

Since many sites are now requiring this prior to IPPE and/or APPE, all students must also have a 2-Step Tuberculin Skin Test completed and the results posted to CastleBranch by 4/21/17. The necessary form is attached to this e-mail. Your HIPAA Training module must be retaken and posted as well. The HIPAA training site will not allow you to redo this until it has been one year since you last did it, but you must do it when you have passed the one year point. While you are looking in your CastleBranch profile, please update any other items that require updating. Your PA Pharmacy Intern License, CPR card (P3 students), HIPAA training, and all immunizations should be complete and uploaded to CastleBranch. Failure to have all of this taken care of by the due dates may prevent you from starting your rotations. This could have a serious effect on your rotation schedule. If you have any questions regarding your CastleBranch account please contact Ms. Anna Schmotzer at [email protected] or (412) 624-8186.

(Do not schedule a urine drug screen at this time. If one of your rotations requires a urine drug test, please notify Ms. Schmotzer one month before the rotation and she will send you instructions.)

Thank you for your prompt attention to this matter.

Sue Skledar, BS Pharm, MPH, FASHP Director of Experiential Learning and Continuing Professional Development University of Pittsburgh School of Pharmacy 921 Salk Hall 3501 Terrace Street Pittsburgh, PA 15261 Phone: 412-648-1504 Fax: 412-383-9996 E-mail: [email protected]

James J.
