We see children that are uninsured, underinsured, and Medicaid or Badgercare. </p><p>Kindergarten students will need a physical, along with vaccinations 6th grade student requires a physical, along with vaccinations 9th grade students require a physical 12th grade students require a meningococcal vaccine </p><p>Please call 815-971-5800 option 5 for appointments</p><p>This is a FREE and CONFIDENTIAL service.
