<p> NORTH LONDON HOSPICE</p><p>JOB DESCRIPTION</p><p>JOB TITLE: Registered Nurse </p><p>Palliative Care Support Service (PCSS)</p><p>GRADE: Band 6</p><p>HOURS: Bank: Min 2 – Max 9-hour shifts as required</p><p>RESPONSIBLE TO: Triage Development Manager</p><p>ACCOUNTABLE TO: Associate Director - Clinical Services</p><p>JOB SUMMARY:</p><p>The post holder will be a member of the North London Hospice (NLH) Palliative Care Support Service (PCSS). The post holder will be responsible for implementing and maintaining high standards of nursing care for individual patients in their own home, providing skilled nursing care, treatment, advice and support to patients and their carers.</p><p>To fulfill this role, the post holder will be working alongside the wider statutory services community team and will be expected to work shift duties including night shifts.</p><p>All nurses must act in accordance with the Nursing and Midwifery Council Code of Conduct.</p><p>KEY RESPONSIBILITIES:</p><p> To participate in the planning, assessment, implementation and evaluation of nursing care, supplementing the care provided by the District Nursing Team to enable the patient to continue to be cared for at home.</p><p> To use evidence-based practice to develop and maintain a high quality of patient care.  To provide advice, support and respite care for the carers and family according to their needs and the level of intervention required during the shift.  To communicate and liaise with medical and nursing colleagues, providing accurate reports of any changes in the condition of the patient.</p><p> To maintain clear, accurate and concise records in the District Nurses notes and verbal handover to PCSS Coordinator. </p><p> To participate in audit documentation as requested.</p><p> To administer medication according to the care plan and be aware of the current drugs and therapies used in the treatment of pain and other symptoms.</p><p> To set realistic goals for each of the identified needs and problem following liaison with professional and carers.</p><p> To critically evaluate nursing care which has been given and liaise with the district nurse if the care plan requires altering.</p><p> To act as the patient’s advocate in communicating information between the patient and members of the multi-professional team. </p><p> To demonstrate insight into the many cultural aspects of life and death, and the implications for nursing care.</p><p> To demonstrate knowledge of the grieving process and recognise normal and abnormal grief reactions.</p><p> To assist in the orientation of new members of staff to the PCSS service. </p><p> Participate in formal and informal educational programmes.</p><p> To adhere to NLH policies, procedures and protocols.</p><p> To adhere and to the NMC Code of Professional Conduct and be conversant with the Scope of Professional Practice.</p><p> To attend the induction programme, team meetings and training sessions as requested by PCSS Coordinator.  To keep up to date with current developments in nursing and palliative care.  To make the PCSS Coordinator aware of availability as requested.</p><p>All information concerning patients and staff must be held in the strictest confidence and may not be divulged to any unauthorised person at any time, unless to do so is in the best interest of the individual. In this instance the post holder should be appropriately advised by the PCSS Coordinator. A breach of confidentiality will result in disciplinary action being taken in accordance with the Hospice's disciplinary procedure and may lead to dismissal.</p><p>This job description is not exhaustive.
