<p> Level of Implementation Scale</p><p>Fully Not Yet Initiated In Process Positive Behavior Supports & School-based Behavioral Health Implemented 1. All programs or resources that are intended to improve students’ behaviors are inventoried and organized into three levels or tiers.</p><p>2. All school faculty members receive training in school-wide positive behavior supports.</p><p>3. All school faculty are trained in screening, identification and appropriate referral for students with mental health problems (i.e., identifying a problem exists v. diagnosis).</p><p>4. All school personnel receive training in de-escalation of challenging behavior and/or identified school staff members are trained in crisis intervention.</p><p>5. School staff members are trained in the relationship between effective instruction and behavior and in how to implement strategies to ensure academic engagement and to reduce disruptive classroom behavior.</p><p>6. Identified school staff are trained in functional assessment of behavior.</p><p>7. Scientifically-based program(s) are in place to support development in the five researched-based elements of social-emotional learning: self-awareness; social awareness; self-management; relationship management; and responsible decision making.</p><p>8. Effective school team(s) (e.g., SWEBS core team, resource coordination team, student centered team, etc.) exist(s) to conduct data analysis, screening, referral, and consultation for student services throughout the 3 Tiers. 9. The school conducts universal screening of emotional, social, & behavior adjustment at all grade levels.</p><p>10. Individual student observations, functional behavior assessments/functional cognitive behavior assessments and/or functional academic assessments or other assessments as necessary are conducted by appropriate staff for the purpose of establishing intervention protocols..</p><p>11. School staff implements research-based interventions to address students’ behavioral difficulties</p><p>12. A continuum of services including in-school behavioral health treatment is available for students with emotional, social, and behavior disorders. 13. The evaluation team uses other assessment procedures as determined by an individual student’s needs in conducting a comprehensive evaluation of student eligibility.</p>
