<p>Date: ___/___/______Store# ______Completed By:______Forklift# ______Hour Meter: ______</p><p>Forklift Inspection Report Instructions: This form should be completed Monday morning of each week and turned in to the store manager after completion. After the form is reviewed by the manager and all problems acknowledged, it should then be put in a file on the lift to be kept for at least six weeks then moved to a permanent file. **NEVER WORK OR STAND UNDER RAISED FORKS WITHOUT PROPER LOCKING DEVICES!!** **ALWAYS CHECK THAT THE PARKING BRAKE IS ON BEFORE INSPECTION!!**</p><p>Yes/No </p><p>Hydraulic Cylinders – Free from leakage/damage on the lift, tilt, attachments of the cylinders.</p><p>Undercarriage – Inspected for debris such as pallet bands, shrink-wrap, etc. Debris removed. (Do not work or stand under raised forks!) Tires/Wheels – free of large cuts and missing lugs.</p><p>Hydraulic Fluid – level checked and sufficient</p><p>Transmission Fluid – level checked and sufficient</p><p>Engine Oil – level checked and sufficient If oil level is low, please note level before adding oil: ______If oil appears to be a milky or an unusual color please note:______</p><p>Brakes – functioning properly If pedal goes all the way to the floor when you apply the service brake that is the first indicator</p><p> that the brakes are bad. Brakes should work in reverse, also. </p><p>Parking brake – functioning properly The truck should not be capable of movement when the parking brake is engaged. </p><p>Propane Tank - Is the tank guard bracket properly positioned and locked down? </p><p>Propane Hose – Free from damage It should not be frayed, pinched, kinked, or bound in any way. </p><p>Propane Odor - If you detect the presence of propane gas odor, turn off the tank valve and report the problem. </p><p>Engine Coolant - Visually check the level. Note: Never remove the radiator cap to check the coolant level when the engine is running or while the engine is hot. Stand to the side and turn your face away. Always use a glove or rag to protect your hand.</p><p>Seat Belts - Do they work? </p><p>Safety Tread –Missing or torn? </p><p>Back Up Alarm - functioning properly?</p><p>List any other problems found with the truck. ______8/28/15</p>
