<p> TRANSIT CUSTOMER SERVICES – TRANSIT SERVICE CENTERS GOLDEN PASSPORT ELIGIBILITY REGUIREMENTS</p><p>All Golden Passport applicants must be a resident of Miami-Dade County. Applicants 65 years and older must present the following documents:  Proof of age  Picture identification (State of Florida Driver’s License/Florida ID, current U.S. Passport, US Alien Registration Card) is required to process the application.  Proof of Miami-Dade County residency (utility - light, phone, water bill, voters registration)  Provide Social Security Number (Copies of all documentation must be attached to application)</p><p>Applicants under 65 years must present the following documents:  Proof of Social Security Benefit (Submit a current copy of status letter showing current and continuing benefits from Social Security Administration)  Picture identification (State of Florida Driver’s License/Florida ID, current U.S. Passport, US Alien Registration Card) is required to process the application.  Proof of Miami-Dade County residency (utility bill - light, phone, water, voters registration) Golden Passport customers 64 and under must re-qualify yearly or as deemed by Miami-Dade Transit. Customers will be required to provide a current Social Security status letter for renewal. </p><p>LOST OR STOLEN GOLDEN PASSPORT Lost Golden Passport – A fee will be charged for lost Golden Passport. The charges are as follows:  1st Replacement fee $5.00 per card  2nd Replacement fee $20.00 per card  All further replacements $50.00 per card Stolen Golden Passport – A one time fee will waived to replace stolen card, when a copy of the police report listing the Golden Passport as stolen is submitted within 30 days of the incident.</p><p>Lost or stolen golden passport cards can be processed at any Transit Service Center listed below or by mailing the replacement fee (NO CASH, money orders) or police report to Miami-Dade Transit, Golden Passport, P.O. Box 01-9005, Miami, FL 33101-9005.</p><p>PARKING: Golden Passport customers are required to purchase a monthly parking permit or pay the daily rate to park at Metrorail Stations. Parking permits can be purchased at the locations listed below or any Team Metro office. Golden Passport customers must present their Golden Passport card in person at time of purchase.</p><p>The Golden Passport is for your use only. Misuse of a Golden Passport will result in forfeiture of its use for one year.
