Box 9620, Naperville, IL 60567</p><p>I hereby apply for membership to the Joliet Officials Association - Basketball Division and, if accepted, agree to actively participate and adhere to the Association’s constitution and bylaws. I understand that the first year I will be considered a Level 1 Associate without voting privileges and any future promotions within the association are based on the results of an approval vote by the Executive Board of the Basketball Division. The second year I will be put forth for consideration as a full member with all entitled member privileges provided I maintain a status of “member in good standing”. New Associates who have attained a “C” rank with the IHSA will have a one year period as a Level 1 Associate and will be put forth for consideration to full membership after the first year. I also understand I must remain a “member in good standing” to maintain associate status and achieve full member status by attending a minimum of 3 meetings per year and promptly paying the annual dues. I also understand that as a full member or associate, I am not entitled to game assignments at any level from any assignor affiliated with the association.
