<p>Chair’s Initiative for a Sounder Future</p><p>Application Guidelines and Narrative</p><p>The Chair’s Initiative for a Sounder Future grants are made possible by the passing of Senate Bill 5152 which created special license plates for the Seattle Sounders FC. The bill was requested by Lieutenant Governor Brad Owen and was signed into law by Governor Jay Inslee in May 2013. The majority of the funds generated from the sale of Sounders FC license plates will support Mentoring Works Washington (MWW) and our efforts to increase the overall number of youth paired with a mentor in Washington State. To that end, MWW is introducing this new grant opportunity for mentoring providers. The goal of this grant program is to increase the overall number of youth matched with a mentor. The maximum grant award is $5,000. Successful applicants will be required to provide a 50 percent match. For example, if a program is awarded $2,000 they will be required to match $1,000. The match cannot be in-kind.</p><p>Who should apply:</p><p>Applicants must be partner providers of Mentoring Works Washington. Visit our website for more information on becoming a member. </p><p>Applicants must provide mentoring to school-aged youth in Washington State. </p><p>Applicants must have a minimum of one year of successful program operation.</p><p>Applicants must serve 10 or more youth.</p><p>Applicants must provide 1:1 or small to medium group mentoring.</p><p>Grants are made annually. Programs may receive funding in two consecutive years and will then be required to take one year off before re-applying.</p><p>Note: Applicants whose proposals are accompanied by a letter of support from a member of the board of directors of Mentoring Works Washington will receive additional consideration.</p><p>1 How to Apply:</p><p>Attach the completed Application Cover Sheet to your narrative. The narrative portion should be no more than three pages, single spaced with 11pt font. Email as one pdf to Janet Heubach at [email protected]. Applications are taken on a rolling basis.</p><p>A complete application must include the cover sheet and a narrative that addresses each of the questions below. </p><p>1) Organizational Background. Include mission statement, history of successes, area and population served, role of volunteer mentors. </p><p>2) Needs Statement and Population Served. Identify the specific needs your program addresses. Are the youth served recipients of any of the following services paid for or provided by Washington State's Department of Social and Health Services (DSHS)? If so, please estimate the number of youth. </p><p>• Developmental Disabilities</p><p>• Foster Care</p><p>• Juvenile Rehabilitation Services </p><p>• Child Protective Services (CPS) </p><p>• Alcohol and Substance Abuse</p><p>• Homeless </p><p>• Mental Health Issues/Depression</p><p>• Food bank and/or food stamps </p><p>• Free/Reduced Lunch Program</p><p>3) How will the funds be used to increase the overall number of mentor matches?*</p><p>4) Demonstrate your capacity (using specific examples and data) to meet grant requirements to increase number of matches.</p><p>5) How will you fulfill the 50% match requirement? (Match does not need to be new dollars but cannot be in-kind.)</p><p>*To determine your number of mentoring matches please average the number of matches you had 12 months ago with the number of matches you have today. </p><p>2 Chair’s Initiative for a Sounder Future MWW Board Letter of Support</p><p>Thank you for your interest in nominating a Mentoring Works Washington member organization as a potential recipient of a Chair’s Initiative grant. The purpose of these grants is to increase the overall number of mentor and mentee matches across Washington State.</p><p>Nomination Process/Letter of Support – MWW Board members are encouraged to nominate programs with which they are familiar for these special grants. Nominated organizations will be required to complete the grant application (attached) and will receive special consideration when the application is combined with a letter of support. </p><p>MWW board members are welcome to nominate more than one program. Board members who are staff members of a mentoring program may not nominate their own program. However they may solicit a letter of support from a fellow MWW board member. </p><p>Board members will be asked to address the following questions in their letters of support: a. What impresses you most about this program and what distinguishes it as worthy of your nomination? b. Describe your connection/relationship to the program. c. Can you help this program fulfill the 50% match requirement and if so, how? d.
