Shift Differentials: $2.00/evening (3:00pm-11:00pm) $3.50/night (11:00pm-7:30pm) for 621-025</p><p>Med Surg, ER and CCU only, rates are not cumulative. Not to include OR nurse</p><p>4. On-call: On –call is at $6.00/hr for provider to wear a pager and be within 30 minutes or 30 miles of the facility. Call back will be at the applicable hourly rate with a one hour minimum.</p><p>5.Call Back: Call back will be at the applicable hourly rate with two hour minimum</p><p>6. Holidays : The negotiated overtime rate for all holidays recognized by the Federal Government that are worked.</p><p>6. Workweek : Sunday (12: 00am) through Saturday (11: 59pm)</p><p>7. Background Investigations : Includes license and certification check, criminal background check done by INFO LINK, Criminal Investigations, sex offender investigations, OSG, ORG (medicare, medical fraud history). Contractor may also be required to provide criminal background check for the personal performing services under this contract. The Government reserves the right to perform an independent criminal background check. Specific requirements will be specified by and submitted to the ordering activity at the task order level.</p><p>8. Prices awarded are based on commercial non-catalog pricing and are subject to clause AS3014 Economic Price Adjustment/ Price Reduction. The base Index or Factor ( BIF ) for determining price adjustments annually is identified as March 2008, representing the Consumer Price Index-All Urban Consumers (CPI-U), not seasonally adjusted, U. S. City Average, U. S. Department of Labor Series ID CUUR0000SEMC / CUUS 0000SEMC for 308.726</p><p>9.
