<p> JOIN US AS A VENDOR FOR THE UPCOMING SUMMER FESTIVAL IN ROCKWELL PARK AUGUST 16TH, 2014</p><p>PRESENTED BY </p><p>SPONSORED BY </p><p>Dear Artist, Crafter, Business Owner, or Community Organization:</p><p>The West End Association invites you to be a part of the Third Annual Rockwell Park Summer Festival. The Festival will be held at beautiful, historic Rockwell Park on Saturday, August 16, 2014. There is a rain date of Sunday, August 17, 2014 if needed. There will be live music, arts and crafts, displays, games, food, and many vending opportunities.</p><p>Act quickly to reserve your booth space by returning the enclosed vendor contract along with your payment. Vendors must provide their own booths or other accommodations (tables, trailers, etc.). </p><p>The Connecticut Department of Revenue Services requires you to obtain a Sales and Use Permit if you will be selling at the event. You must display the permit prominently in your booth or on your table. For more information, please visit www.ct.gov/drs.</p><p>Food vendors only:</p><p>1. Permits are required for vendors who prepare food on site (no permit required for pre-packaged items). Two Certificates of Liability Insurance are required with the following named as additional insured and certificate holders: West End Association, 64 West St., Bristol, CT 06010 AND the City of Bristol Parks and Recreation Department, 2nd Floor, City Hall, 111 North Main St., Bristol, CT 06010. 3.
