<p> Big Friend –Little Friend, Inc Release and Acknowledgement</p><p>IN ORDER FOR YOUR CHILD TO PARTICIPATE WITH THE NATIONAL FIELD ARCHERY ASSOCIATION A PARENT OR LEGAL GUARDIAN MUST COMPLETE THE FOLLOWING:</p><p>Child(ren)s Name (please print):______</p><p>______</p><p>______</p><p>Participant, or parent or legal guardian acting on behalf of participant (hereinafter participant), understands the NFAA will take place at an inside range where several youth will be learning the fundaments of archery. Participant understands that archery is an inherently dangerous activity and can involve risk or injury. </p><p>_____(initial) I hereby give permission for my child(ren) to participate with the NFAA </p><p>. Because of the risks involved in participating in these activities participant recognizes the importance of the instructions regarding the rules of the event. Participant agrees to obey such instructions. In consideration of Big Friend – Little Friend, Inc permitting participant to participate and engage all activities, participant hereby voluntarily assumes all risks associated with learning how to shoot a bow/arrow, on behalf of his/herself and his/her child/ward and his/her personal representatives, heirs, and next- of-kin. Participant agrees and does hereby release, waive, forever discharge, covenant not to sue and hold harmless the Big Friend – Little Friend, Inc, its Board Members and employees from and against all liability for claims, demands, costs or suits for any bodily injury (including death), personal injury and/or property damage of whatever kind arising out of, or resulting from participant’s participation at the NFAA and/or other parts of this program.</p><p>Participant acknowledges and states that participation in these activities is entered into as a free and voluntary act with full and complete knowledge of the risks involved.</p><p>Each child will be accompanied by an adult.
