The full $160 registration fee will be returned to the registrant’s via check within 30 days of the request. To complete your request please email your answers to the 5 questions below in the Request for Refund/Credit Form to [email protected]. </p><p>After 11/2/12, a request for credit will be considered for anyone injured during league play that, by doctor’s orders can no longer participate for the remainder of the league. In order to be eligible for the credit, the player’s injury must be reported to the Empire League Representatives immediately. $60.00 of the registration fee is non-refundable and games are prorated at $10 a week. The balance will be returned to the registrant in form of a gift certificate within 30 days upon approval. No credit will be awarded to any requests placed after 11/25/12.</p><p>For example: 1. A registered athlete is injured during play in week two. 2. They report the injury to the Empire Representative that night. 3.
